It's still all a bit of wizardary in most cases, built up by hack and try methods over the years.
I think that there are rough principals that can be applied to most jobs but since every engine is built slightly different and its intended applications can vary wildly this will also vary the tuning methods, so a different approach will probably be needed on a case by case situation.
Some people swear by a rough casting surface rather than the smooth approach as it helps to mix the air and fuel better than a smooth surface, while other tuners swear by the smooth surface approach to get the air in as fast as possible.
Not everybody has an airflow bench they can put heads into and try until they get the best possible gas-flow, also a super-high level of comprehension around fluid dynamics would help.
All in all if your considering any engine work be wary of anyone who claims their method is 100% and read up / find somone with as much experience as possible :)
By the way.. port polishing is a little different than re-shaping the actual ports by filling / welding / cutting, this is less likely to do massive changes to the engine.
